Ball Hospital/Clarian/CHS Insurance

We are aware that the Ball Memorial/Clarian/CHS vision insurance is changing on January 1, 2010 from Vision Service Plan (VSP) to United Healthcare Vision. We have looked at the information regarding the United Healthcare Vision plan to decide if participation in the plan is a viable option for Family Vision Care.

 

After careful and extensive review, we have determined that we cannot participate in this plan for several important reasons:

·         The glasses available through the plan would not be of the same quality that we now provide you.

·         Our ability to determine which laboratory can best make your glasses would be eliminated, as all glasses must be made at a specific laboratory in Baltimore, MD.

·         It would not be possible to continue our normal 2 year warranty on your glasses.

·         It would be impossible to provide the same highest quality eye examination for the fees they are willing to pay us.

 

GOOD NEWS!!!

 

The good news is that we have taken steps to help make it possible for you to remain a patient here at Family Vision Care:

·         We will extend a savings on the examination when we don't file insurance for you.

·         We will offer a savings on your glasses. 

·         We will provide you with the itemized statement needed to file for your out-of-network benefits.

·         Please call our office to speak with our insurance specialist for details.

Also:

 

·         The new plan offers very reasonable out-of-network benefits for examination and glasses.

·         The contact lens out-of-network benefit is $105.

 

Our mission remains the same:  to provide you with the very best examination and highest quality eyewear.  This means using our state-of-the-art examination instruments and highly trained doctors to carefully monitor your eye health as well as using lenses, frames, and contact lenses of superior quality.  We will not compromise on that promise.
